Lewis County Health Department is a Medicare/Medicaid certified, state licensed agency and is currently a network provider for many commercial insurance companies. Home health care is provided under the direction of a physician.
The goal is to assist fellow citizens of Lewis County to remain in their homes.

To Qualify
Services for Qualifying Residents
Services are provided to Lewis County residents who meet Medicare, Medicaid, or private insurance qualifications.
Skilled nursing care by a Registered Nurse
Home Health Aide services, including assistance with bathing
Physical Therapy
Occupational Therapy
How Referrals Work
If you feel you or someone you know could benefit from home health services, please contact your physician and a referral can be set up. If being discharged from the hospital, a patient has the option for all available home health services depending upon payor source.
Private Pay Caregiver List
Call Lewis County Health Department for an in-home caregiver list. These individuals are not employed by Lewis County Health Department, but are known to provide private pay services in Lewis County.
